Titre : UTILITY OF SCORES IN THE ASSESSMENT OF DIFFICULTY IN LIVER ONCOLOGICAL SURGERY Type de document : thèse Auteurs : KTAMI NOURELIMANE, Auteur Année de publication : 2022 Langues : Anglais (eng) Mots-clés : Liver cancer surgery Scores of difficulty Laparoscopic liver surgeryOpen liver surgery Chirurgie du cancer du foie Scores de difficulté Chirurgie laparoscopique جراحة سرطان الكبد درجات الصعوبة جراحة الكبد بالمنظار
du foie Chirurgie ouverte du foieRésumé : We are reporting 95 cases out of 248 initial cases collected in the department of oncological
surgery of the national Institute of Oncology in the period between 01/2018 and 02/2022,
treated for several types of tumors with the following proportions :[ 19.6% KCVB / 11.3%
CHC/ 48.5%MHCCR / 6.2% CKPH / 5.2% C autres / TB 4.1% ]
Our main purpose is to assess the utility of the available scores in predicting the difficulty of a
liver tumor surgery so as to use scoring in daily practice while deciding the best strategy for
each case .
In our study the male-to-female ratio is :
The scores that have been applied on our selected cases are : IMM(kawaguchi) -
HASEGAWA - HALLS - and the minor/major classical score .
The Judging criteria were intraoperative Blood loss - Surgical time - Major complications
(≥ III Clavien-Dindo )
The significant increase in difficulty level was associated with the
IMM/HALLS/HASEGAWA scores , only correlating the Blood loss as a judging criteria
with a p value of < 0.001 for IMM and Hasegawa scores and 0.008 for Halls score.
No score was significantly correlated to the Surgical time and the postoperative major
complications .The minor/major score allowed us to predict the major complications that are
≥ IIICD with a p value of 0.009.
