Titre : | BLOC MAXILLAIRE BILATERAL PAR VOIE SUPRA ZYGOMATIQUE DANS LA CHIRURGIE DE LA FENTE LABIO PALATINE CHEZ L’ENFANT : VERS UNE ANESTHESIE SANS MORPHINIQUE | Type de document : | thèse | Auteurs : | Yousra GUELZIM, Auteur | Année de publication : | 2021 | Langues : | Français (fre) | Mots-clés : | Bloc maxillaire supra zygomatique Anesthésie Analgésie Morphine | Résumé : | Introduction: Cleft lip and palate repair surgery is painful and highly risked in terms of
respiratory complications, which are emphasized with morphine-type administration.
Maxillary nerve block loco-regional anesthesia is an alternative technic to conventional
analgesic approaches.
Our study objective is to determine Bupicavine efficient minimum dose to be injected « In-
plan» through suprazygomatic block approach while ensuring method’s safety and efficiency
Material & method: This prospective randomized triple-blinded study has been realized
in the main operating unit of Rabat’s Children Hospital’s facility, over a 12 months period [Jan.
20 – Dec. 20]
Patients treated with suprazygomatic block approach that have been split into 2 equal
groups (G1 & G2 - 30 people each), with the following ventilation: G1 bupivacaine 0,125%
& G2 bupivacaine 0,25%.
Main assessment criterion was postoperative pain measurement through FLACC score
method.
Results: Suprazygomatic block technic was successfully executed on every patient of both
groups. No differences have been identified within demographic DATA nor clinical features.
Intraoperatively, in both groups, heart rate and mean blood pressure have decreased in
comparison to surgical incision momentum (p=0,039 et p0,001)
Postoperatively, analgesia duration within G1 was at 23,6 ± 1,12 hours vs. 19,73 ± 5,39
within G2 (p= 0,026).
In parallel, Morphine total consumptions were at 93 ï›0-200ï ïg and 200 ï›0-240ï ïg
(p=0,049). No serious complications have occurred
Conclusion: Usage of 0,125% Bupivacaine solution provide a qualitative post operation
anesthesia equivalent and comparable to the one gained with 0,25% Bupivacaine. | Numéro (Thèse ou Mémoire) : | M1002021 | Président : | Salma ECH-CHERIF EL KETTANI | Directeur : | Alae EL KORAICHI | Juge : | Aziza BENTALHA | Juge : | Nawfal FEJJAL |
